Student Emergency Contact & Medical Form

Each year, students must submit a new Student Emergency Contact & Medical form. This form must be completed for each student. Siblings must have their own individual forms completed.

Medication Forms

These forms must be submitted to the school office before any medication can be administered to your children. All medications must be provided by parents and be in their original packaging.

All documentation is required to be submitted to the Archē Classical Academy Office BEFORE students may attend school. Students will not be permitted to attend school without submitted documentation.

Immunizations & Exemptions

The Commonwealth of Pennsylvania requires students to submit immunization records for school attendance. However, Archē Classical Academy recognizes exemptions permitted under state law. Parents/guardians may request exemption from immunization requirements for medical reasons, religious beliefs, or strong moral/ethical convictions. Families claiming exemption, please complete and return the Statement of Exemption to Immunization Law Form to the office or email it to [email protected].
